You can fill out your forms electronically and send them to us safely and securely online at the email address office@drcito.com. These forms must be filled out prior to all new patient visits. Please choose the electronic option, and please access the forms using Google Chrome, as this is the most secure browser on the web.
By completing your child’s paperwork before your visit, you will give our pediatric dentists and team a chance to prepare for your visit, review your child’s medical history and plan their treatment. Your child will spend less time in our waiting room and more time getting a healthy and beautiful smile for a lifetime.
